Police released information today saying drugs and alcohol were determined to be contributing factors in a fatal motor vehicle crash reported over a month ago in South Maui.

The incident was reported at 7:58 p.m. on Thursday, June 20, 2013, on Mapu Place, about 90-feet east of the Mililani Street intersection in the Maui Meadows subdivision.

Maui police say 36-year-old Sirius Fazao Pereira of Wailea was initially transported to the Maui Memorial Medical Center in critical condition, but succumbed to his injuries and died on Thursday, July 4, 2013.
